Your Evvy Clinical Care purchase may be eligible for reimbursement. If you have questions about eligibility, the best thing to do is to contact your plan administrator prior to purchasing. We see the majority of people able to use their HSA / FSA card or get reimbursement following the below instructions. However, since every insurance plan is different, we cannot guarantee reimbursement.
If you need an itemized receipt to submit for reimbursement to your HSA / FSA after purchasing, please reach out to us directly through the blue chat button on the bottom right of your screen or email support@evvy.com. This itemized receipt can be generated once your consult has been completed and your treatments have been determined.
Please note that at this time, Evvy-affiliated providers are unable to complete any additional forms on your behalf that may be required by your HSA / FSA for reimbursement.
